EMPLOYER & EMPLOYEE RESOURCES
Find your doctor. Check your prescriptions.
Compare the providers and medications that matter to your team before choosing a health plan. Start with the exact carrier, network, plan year and prescription drug list shown in your proposal.
Start here
1. Match your plan. Use your proposal or ID card to identify the network. The carrier name alone is not enough.
2. Set your location. Choose the city or ZIP code where you will receive care. Check each doctor and each hospital separately.
3. Confirm your results. Ask the provider and carrier to confirm participation in the exact network and practice location, and whether the provider is accepting new patients. Directories and coverage can change.
Doctor lookup
Open your carrier below and select the network that matches your proposed or enrolled plan. Most directories offer a guest search; enrolled members can sign in for plan-specific results.
Premera Blue Cross
Open Premera’s doctor directory
Association Healthplans
For a plan using Heritage, select Heritage. For a plan using Heritage Prime, select Heritage Prime. Use BlueCard PPO for an applicable out-of-area search; confirm your plan’s out-of-area rules.
Small Group plans have a metal tier in the name, such as Bronze, Silver, Gold or Platinum.
Small Group Choice PPO plans
Select Heritage and Dental Choice when that is the network identified in your proposal.
Small Group Balance EPO plans
Select Heritage Signature and Dental Choice when that is the network identified in your proposal. Check hospitals as well as individual doctors for participation in your plan.
Kaiser Permanente Washington
Open Kaiser Washington’s doctor directory
Access PPO
Select Access PPO. Set your location before searching.
For Access PPO plans, the proposal may also provide access through First Choice Health and Aetna Signature Administrators. Match the network and service area to your Access PPO plan.
Core HMO
Select Core. Set your location before searching. Use the Kaiser Permanente Core network directory for Core HMO plans.
Regence
Open Regence’s doctor directory
For a Preferred plan, search for Preferred and choose the Washington employer-sponsored network. For an applicable out-of-area search, choose National BlueCard PPO. If the proposal says Essential or another network, select that exact network instead.
Asuris
Open Asuris’s doctor directory
For a Preferred plan, choose the Washington employer-sponsored Preferred network. Use National for an applicable national-network search. Match any other network to the proposal.
Nationwide / Allstate Health Solutions
Aetna Signature Administrators directory | Other Nationwide / Allstate network options
For quotes labeled Aetna Signature, use Aetna Signature Administrators. Nationwide quotes may also use First Choice or Cigna PPO. Select the network printed in your proposal; a provider’s participation in one network does not establish participation in the others.
UnitedHealthcare
Open UHC’s doctor search | UHC member account
Level-funded Choice Plus: In the guest medical directory, choose Employer and Individual Plans → Level Funded Health Plans → your state → Choice Plus, when those options appear. For fully insured/small-group plans, follow the corresponding plan category and select the exact network, such as Choice or Choice Plus. Do not use the level-funded selection for every UHC plan.
Dental & vision
Delta Dental of Washington — find a dentist: For BHT plans described as covering all Delta networks, choose All. Otherwise match the network in your plan.
VSP — find an eye doctor: For a VSP Choice plan, select Choice under advanced settings. Use a different network only when your plan specifies it.
Prescription formularies
A formulary is a plan’s covered-drug list. Search the exact medication, strength and dosage form. Check its tier, prior authorization, step therapy, quantity limits, specialty-pharmacy requirements and pharmacy network. A listed drug is still subject to your plan’s coverage rules and cost sharing.
Premera
2026 Small Group formulary
2026 Large Group formulary
Plan-year check: These supplied PDFs are for 2026. For a 2027 effective date, confirm the 2027 formulary before making a decision. For an Association Healthplan, confirm which drug list the proposal uses; employer size alone does not determine the list.
Kaiser Permanente Washington
Washington Small Group formulary
Washington Large Group four-tier formulary
Choose the list specified by the plan and check the effective date inside the document. Confirm the applicable formulary for an Association Healthplan.
Regence / KAVI
Regence / Prime Therapeutics medicine search
Select the correct plan and year. For KAVI plans using the six-tier metallic formulary, select that list. Other Regence plans may use a different list.
Asuris
Use the pharmacy information in your Asuris member account or the formulary linked in the proposed plan’s SBC. Ask Sapper to confirm the drug list before using a Regence or other carrier’s list.
Nationwide / Allstate — check the pharmacy administrator
myCigna prescription resources | Allstate Health Solutions member resources
Some Nationwide / Allstate SBCs in our plan resources direct members to myCigna for prescription coverage. Use myCigna only when the proposed plan’s SBC identifies Cigna for pharmacy benefits. Existing members can sign in for their plan’s covered medications and pharmacy information.
Before enrolling: If your Nationwide quote does not name the pharmacy administrator or formulary, ask Sapper to confirm the exact drug list for that quote. An Aetna Signature or First Choice medical network does not identify the prescription formulary.
UnitedHealthcare — match the Prescription Drug List (PDL)
myuhc — plan-specific prescription benefits | UHC pre-enrollment plan resources
UHC level-funded SBCs in our resources direct members to myuhc. Sign in for your plan’s drug coverage and costs. Before enrollment, use the drug list named in the quote or SBC and ask Sapper to confirm it.
UHC small-group SBCs in our resources identify different lists, including Essential, Essential with Specialty Medication Cost Share (SMCS), and Advantage. These are plan-specific options. Choice Plus is a medical network, not a formulary name. Match the PDL, pharmacy network and year to your plan.
Need help matching a plan?
Tell us the employer name, carrier, plan name, effective date and the network or drug-list name shown in the proposal. We can help identify the correct resource before you select benefits.
Employers can share this resource with employees so they can check their own providers and medications. Please keep personal medical details out of employer group emails.
Resource review: October 30, 2026. Carrier directories and formularies may change. Plan documents govern coverage; directory and formulary searches do not guarantee payment.
